2013 WAPC Callon-Leonard Award
The WAPC Callon-Leonard Award is the highest individual honor granted annually by WAPC in recognition of its founders, Helen F. Callon, RN, CNM and Thomas A. Leonard, MD. The Callon-Leonard Award is presented to an individual who has made a significant contribution to perinatal care in the State of Wisconsin.
Please review the following guidelines before submitting your nomination. Nomination forms are due to the WAPC office by October 1, 2012, for the 2013 award. Click here to download a nomination form. - The recipient does not have to be active in the State at the time he/she receives the Callon-Leonard Award.
- The recipient does not have to be a member of WAPC.
- Any WAPC member can submit a nomination for the Award. The nomination must be submitted on the appropriate form to be considered. The form must include a brief resumé of nominee's credentials, a description of the candidate's contributions to perinatal care, and why the candidate should receive the award.
- Nominations will be reviewed by the Callon-Leonard Award Committee. The committee will make the final decision regarding the award recipient. It is not essential that the Award be granted each year if a suitable recipient is not available.
- The Callon-Leonard Award Committee shall consist of the three past-presidents of the Association and the three past recipients of the Award. The Immediate-Past President shall serve as Chairperson.
- The selection of the award recipient will be announced in the WAPC annual conference brochure.
- The Callon-Leonard Award is presented at the awards banquet at the WAPC annual conference. The committee will choose the person who will present the award to the recipient.
- The completed form can be submitted to the WAPC office by US mail, email or fax by October 1, 2012, for the 2013 award.
- The Callon-Leonard Award Committee and WAPC staff will arrange to have the award plaque prepared.
